Hospice care

Hospice is a unique program to give comfort and individualized care to a patient and their loved ones confronted with a life limiting illness.

When a patient is diagnosed with a terminal illness and all curative measures have been exhausted, comfort and palliative care is provided through hospice. 

Care and services are provided by our highly qualified and experienced professionals in order to serve the needs of a patient and their loved ones during the end of their life.

Our goal

We create an individualize plan of care to meet the physical, emotional, and spiritual needs of our patients. Our interdisciplinary team creates a customized plan suited to each patient with the utmost respect and response to the unique differences with each family or caregiver lifestyle, values, beliefs, and wishes,

We will be of assistance every step of the way to provide the patient and their family with care and resources to get through this trying time. We are available 24/7 through an on-call nurse, as well as our office staff during business hours.
